Runners and athletes. Bone stress is the specialty.

Injury is load exceeding what the tissue could take, and rehab is rebuilding what it can take. That applies to a stress fracture and to an Achilles alike. The difference is in the timelines and the risk, which is why the diagnosis comes first.

Bone stress injuries

Every bone heals differently.

Each needs a specific loading plan, not generic rest.

Tibial stress fracture

Shin bone, the most common running bone stress injury. Posterior medial is low risk; anterior cortex is not.

Navicular stress fracture

A high risk midfoot bone that needs careful offloading and imaging led progression.

Metatarsal stress fracture

Forefoot, common after a training spike. Most are low risk; the base of the fifth and the second are the exceptions.

Femoral neck stress fracture

High risk hip injury. Groin or thigh pain in a runner gets treated as this until proven otherwise.

Sacral and pelvic stress fracture

Often missed and often linked to bone health and energy availability.

Low energy availability and RED-S

The fuel side of bone stress injury, and of a lot of other injuries that keep coming back.

Running and sporting injuries

The rest of the caseload.

Same thinking, different tissue.

Shin pain that is not a stress fracture

Medial tibial stress syndrome, or shin splints. Working out which one you have is the first job.

Achilles and patellar tendinopathy

Tendons adapt to load the same way bone does, just slower and with more patience needed.

Plantar heel pain

Plantar fasciopathy and its neighbours. Load, footwear and calf capacity, in that order.

Knee pain in runners

Patellofemoral pain, iliotibial band pain and the rest. Usually a capacity problem wearing a knee costume.

Calf, hamstring and hip muscle injuries

Strains, and the proximal hamstring and gluteal tendon problems that masquerade as them.

Return to sport after any injury

Football, basketball, court and field sports. The back end of rehab is where reinjury gets decided, so it gets tested, not assumed.
